Account
Card:
Use this form to establish your EECU membership, request to
open an EECU Checking Account, request to open an EECU Money
Market Account, assign/change beneficiaries, or to assign/change
joint owners.
Alumni
Club Membership Request Form: Use this form to sign
up for EECU’s Alumni Club. If you’re age 50 or older and maintain
an aggregate EECU deposit balance of $5,000 or more, you’re
eligible for Alumni Club membership and its special benefits.
Change
of Address Form: If you are moving, please use this
form to notify us about your new address.
Check
Re-Order Form: Use this form to re-order personal EECU
checks.
Deposit/Withdrawal
Form: This is the same form we have in our branches
for making deposits and withdrawals at a teller window. We’ve
included this form here because many members enjoy the convenience
of mailing in their deposits.
Electronic
Services Agreement: Use this form to request EECU On-Line
Internet Banking service, request EZ Pay PC bill payer service,
request a free EECU VISA Check Card or Money Machine ATM Card,
request Touch Tone Teller (banking by phone) authorization code,
or request a new PIN for your existing EECU VISA Check Card
or Money Machine ATM Card.
Express
Transfer Authorization Agreement: Use this form to request
the Express Transfer Service.
Quest
for Education Scholarship Application: If you're a graduating
high school senior and are an EECU member, you're eligible to
apply for one of twenty-five $1,000 college scholarships we'll
award in April, 2003. Hurry, the application deadline is Friday,
November 22, 2002.
Savings
Transfer Authorization Form: Use this form if you want
us to transfer funds you have on deposit at another financial
institution, to your EECU account.
Secret
Savers Account Card: Use this form to establish EECU
membership, and Secret Savers Club membership, for your child
age 10 and under.
Transfer
Form: Use this form to transfer funds between your
different account types (checking to savings, savings to checking,
checking to a loan, etc.). This is the same form we have in
our branches for transferring money at a teller window. We’ve
listed the form here because many members enjoy the convenience
of banking by mail.
